Hospital Discharge Care Coordinator Nurse

Hennepin County's Health Care for the Homeless (HCH) is seeking a
full-time Hospital Discharge Care Coordinator Nurse who has an
interest in, and skill and experience working with, people
experiencing homelessness.

This position will coordinate homeless individuals discharged
from the hospital to homelessness, and act as the primary liaison
between the hospital, HCH's Respite Program, and the shelter
community. They will focus on "transitions management" as
patients who are homeless move into primary care health care
homes, out of inpatient hospitalization, and into shelter
settings and stable housing. They will ensure necessary medical
and social supports are in place to enable success with these
transitions.

HCH provides preventive health and acute care services on a
walk-in basis to adult men and women, youth, children, and
infants. This Hospital Discharge Care Coordinator Nurse will
coordinate with a close team of coworkers (Physicians, Nurse
Practitioners, Physician Assistants, other Registered Nurses,
Licensed Practical Nurses, Medical Assistants, Social Workers,
and Case Management Assistants).

Work hours are Monday through Friday and include working one to
two evenings each week. No weekend, overnight, or holiday shifts
are required. This individual must be able to travel among
shelters and drop-in centers to provide medical services.

The primary duties and responsibilities of this position
include:
•Provide care coordination and referral to ensure seamless
delivery of care and care transitions to improve communication
among health and human service providers and to guide clients and
communities toward an effective use of resources.
•Work with community partners to facilitate the integration of
healthcare and social services and to identify homeless
individuals with health care needs.
•Initiate and lead targeted "in-reach" at assigned hospital and
community site(s) to engage homeless individuals and their
families in order to educate, refer, and connect them to the
services provided by HCH.
•Deliver skilled nursing care and interventions including, but
not limited to, medication and vaccine administration, vital sign
monitoring, dressing changes, and documentation of treatment
progress and patient response in an electronic health record
(EHR).
•Provide direct nursing care to patients including triage,
assessment, screening, and intervention, and carry out orders of
the primary care or other integrated care providers (e.g. medical
respite, behavioral health).
•Serve as a clinic and patient care coordinator for HCH and act
as the primary liaison for the community.
•Coordinate efforts of an interdisciplinary patient care team
and facilitate daily communication between patient care team to
ensure integrated and coordinated care.
•Supervise and lead the day-to-day work of clinical support
staff (LPNs and CMAs) and evaluate the delivery of delegated
care; delegate clearly and directly to care team.
•Facilitate quality improvement (QI) team meetings and report
on-going QI activities to clinic manager; coach and mentor staff
to address continuous QI in site operations, customer service,
nursing care, and adherence to policy and procedures manual.
Best Qualified Candidates will have: •Graduated from an
accredited school of nursing.
•A valid license as a Registered Nurse (RN) issued by the state
of Minnesota.
•Current CPR certification.
•Access to reliable transportation to travel among shelters and
drop-in centers to provide medical services.
•Experience: ◦As a registered nurse providing care to
vulnerable or underserved populations.
◦Working with electronic health records.
◦Using motivational interviewing and health coaching
techniques.

•Ability to: ◦Work collaboratively as part of an
interdisciplinary health care team.
◦Work a flexible schedule during the week (no weekends) –
including evening hours.



Invitations to interview will be based on an assessment of
education and experience. Final candidates will be required to
complete a drug test and a background check.

The top four steps in this pay range are dependent on
professional nursing experience and education.
