Social Worker/Senior Social Worker

THE POSITION
The Long Term Services and Supports and Adult Protection area of the Human Services and Public Health department has an opening for a Social Worker/Senior Social Worker in our Children's Long Term Case Management team. The team's goal is to promote and support the health, safety, and well-being of children with developmental disabilities and related conditions. The applicant for this position will work within an interdisciplinary team that includes mental health professionals, educators, medical professionals, and social workers.

Bilingual candidates in Somali, Spanish, and/or Hmong in addition to English are encouraged to apply.

Social Worker $25.67 - $31.96/ hour
Senior Social Worker $26.10 - $37.60/ hour

The primary duties and responsibilities of this position include:
  • Review records to assist in determining and maintaining eligibility, and understanding the child's needs
  • Provide ongoing case management, including minimum twice a year visitation with each child
  • Assist in the coordination of services to meet the child's physical, educational, cultural, medical, and mental health needs in the least restrictive environment
  • Consult with other social workers, therapists, care providers, parents/guardians, Guardians ad Litem, medical and/or mental health professionals and others about the care needs of the child
  • Utilize and apply person-centered principles and practices in conducting assessments and work on cases
  • Complete documentation in a timely manner; including time reporting, social history, service and placement plans, and Full Team Screenings
  • Comply with federal and state regulations for case management of children with intellectual and developmental disabilities, and follow best practice and professional standards
Best Qualified Candidates will have:
  • For hire as a Social Worker, one of the following:
    • Bachelor's degree in social work, psychology, sociology, human services, or equivalent field
    • Licensed as a Social Worker by the Minnesota Board of Social Work
  • For hire as a Senior Social Worker, one of the following:
    • Master's degree or higher in social work or closely related field with course work emphasis in areas such as social work methods, human growth and behavior, and principles or social welfare
    • Bachelor's degree in social work, psychology, sociology, or human services, and at least 18 semester (27 quarter) of graduate work in the above areas, and three years or more of full-time supervised experience as a social worker
    • Licensed as a Graduate Social Worker by the Minnesota Board of Social Work
  • A valid driver's license and access to reliable transportation to travel within Hennepin County.
  • Meet the federal criteria for Qualified Developmental Disability Professional (QDDP) or have the ability to obtain within 12 months of employment.
    • One or more years of experience working with individuals with developmental disabilities.
  • Experience:
    • Working with individuals with developmental disabilities, physical disabilities, medical conditions, and/or related conditions.
    • Assessing individuals using a person-centered approach.
    • Working with medical assistance state plan, waiver services, and Rule 185.
  • Knowledge of county and community resources.
  • Recognition and respect for culture differences and culturally appropriate services.
