Community Health Worker, Senior - Child Wellbeing

THE POSITION
Public Health Protection and Promotion is seeking a Senior Community Health Worker to work with children birth to age five in Out of Home Placement (OHP). This position is part of an interdisciplinary team of Public Health Nurses, Child Protection Social Workers and Case Management Assistants with a primary focus on child well-being.
 
The Maternal & Child Health/Early Childhood area is a combination of programs that encompass prenatal, infant, early childhood, and adolescent health.

The primary duties and responsibilities of these positions include:
  • Assist the PHNs in administering screening tools to determine developmental and social/emotional progress, assessment of and referral for, primary medical, dental and mental health care for all children birth to five in OHP.
  • Refer children who do not pass the above screening to school districts, state and county funded programs, and private agencies for services; assist in connecting children to medical and dental providers
  • Communicate with parents of children screened to interpret the results to them and discuss services available to the child and family.
  • Document all services provided as well as results of screening and results of follow-up.
  • Follow-up with schools and mental health providers to determine if contact has been established and services are being provided; partner with Child Protection Social Workers
  • Maintain relationships with programs and services used as referral sources.
  • Provide consultation to Child Protection staff regarding early childhood and public health resources available throughout Hennepin County.
  • Work as a team with the other Maternal & Child Health/Early Childhood staff to share knowledge, expertise, and problem-solving on projects involving screening and to institute electronic screening across all of the projects.

Best Qualified Candidates will have:

  • Completion of at least two years of college, in social/behavioral sciences or a field related to the area. Related social service experience may be substituted for education on a year for year basis.
  • Valid driver's license and access to reliable transportation.
  • Experience working with:
    • Children age five and younger and families.
    • A diverse client population and providing culturally appropriate services.
    • Microsoft Office, SSIS, and Electronic Case File management (ECF).
  • A multicultural background and/or bilingual fluency.
  • Ability to:
    • Relate well to families and colleagues.
    • Work with confidential information.
    • Establish and maintain effective working relationships throughout the county.

Invitations to interview will be based upon an assessment of education and experience. Final candidates may be required to complete a drug test and background check. Bilingual candidates must pass a language assessment to qualify for bilingual duties and pay.
